24/7 Complex Care
24 hour care becomes necessary when an individual requires round the clock nursing
or social care, has a serious health condition or complex care needs. We can
provide care, countrywide, alongside family carers, District Nurses, GPs,
Physiotherapists and Occupational Therapists. We have extensive experience of MND/ALS,
spinal injuries, Acquired Brain Injury, COPD, heart conditions, Multiple
Sclerosis and Dementias, for adults over and under 65 years in their own home.
Our service is entirely bespoke to each client and their family, where all needs are met
and expectations often exceeded. Despite any limitations, personalised care and
support is given to our clients to enable them to retain as much independence,
choice and control as possible. We can support our clients to remain living in
their own homes and 24 hour homecare is a viable alternative to nursing home
care.
We setup and manage teams of Personal Assistants that meet our client's particular
needs. Clients are encouraged and supported to be fully involved in choosing their
own team. Personal Assistants are not there just for healthcare needs, they
will be able to support our client to manage their own life. All Personal Assistants
are suitably qualified, will have client specific training, a DBS check, good references
and are fully supervised and supported.
Care Services Managers, a healthcare professional, typically a nurse, visit as often as required,
usually monthly, to ensure all is well, give advice and support. All risk
assessments and care planning are created and updated by the CSM. CSM and management are always involved and available
to our staff and clients 24 hours a day should they be needed.
As part of a 24 hour package, we can attend assessment meetings, make recommendations
and referrals. Funding can be private, Local Authority, Direct Payments or NHS
Continuing Health Care. We always work together and liaise with the wider MDT
in order to ensure the best available treatment and care for our clients.
